丙戊酸镁联合抗精神病药治疗精神分裂症攻击行为

摘要目的评价丙戊酸镁联合抗精神病药治疗精神分裂症患者攻击行为的疗效。方法将60例有攻击行为的精神分裂症住院患者随机分为研究组30例对照组30例。研究组在原有抗精神病药治疗的同时加用丙戊酸镁治疗起始剂量400mg・d-1  1w后增至800mg

1200mg・d-1 对照组继续原有抗精神病药治疗。疗程4w。两组于治疗前及治疗第4w末采用简明精神病评定量表、治疗前及治疗第1、

2、 4w末采用外显攻击行为量表、副反应量表评定疗效与不良反应。结果两组治疗前与治疗后简明精神病评定量表各因子分差异均有极显著性(P<);治疗后两组间比较差异无显著性P>  两组治疗前后外显攻击行为量表评分有极显著性差异(P<) 治疗第2、 4w末两组间比较有极显著性差异(P<) 研究组评分降低更为明显。结论在常觃应用抗精神病药治疗精神分裂症的攻击行为时加服丙戊酸镁疗效更佳

【Abstract】 Objective To assessed the curative effects of thecombination of magnesium valproate and antipsychotics in thetreatment of aggressive behaviors of schizophrenics.Methods Sixtyinpatient schizophrenics with aggressive behaviors were randomlydivided into research and control groups(each n=30).Researchgroupwere given antipsychotics combined with magnesiumvalproate, its beginning dosagewas400mg/d and reached

800~1200mg/d afterone week, and control group with singleantipsychotics for4 weeks.Al l patients were assessed with briefPsychiatric Rating Scale(BPRS),Modified OvertAggression Scale(MOAS) and Treatment Emergent Symptoms Scale(TESS).ResultsThere were significant difference in factor's scores of BPRS between

pre and posttreatmentof the both 2 groups(P<), and no differencebetween the 2 groups after treatment(P>); there was significantdifference in the score of the MOAS between preand posttreatmentof both the 2 groups(P<)and betweenthe 2 groups at the ends of2nd and 4th week(P<).Scores of research group were significantlylowerthan thatof control group.ConclusionCombination ofmagnesiumvalproate and antipsychotics is the best in thetreatmentof aggressive behaviors of schizophrenics.

攻击行为是指有意伤害语言或动作某个体或破坏其他目标如物体的行为包括自杀、 自伤它是精神科临床中较常见的一种行为障碍。James等 1讣为精神病人攻击行为发生率高于普通人群精神分裂症攻击行为发生率高于其它精神障碍且具有突发性和严重危害性。因此对精神分裂症患者攻击行为的控制尤为重要。有很多研究提示抗精神病药联合心境稳定剂能有效控制精神分裂症患者的攻击行为。为此我们应用抗精神病药联合丙戊酸镁对有攻击行为的精神分裂症患者进行了临床疗效观察现将结果报告如下。

1对象与方法

对象选取我院住院的60例具有攻击行为的精神分裂症患者为研究对象。入组标准 (1)符合《中国精神障碍分类与诊断标准》第3版CCMD-

3精神分裂症诊断标准 (2)年龄1856a  (3)简明精神病评定量表(BPRS)

总分38分 (4)外显攻击行为量表修订版 (MOAS) 2总分≥4分

(5)无严重心、肝、肾等躯体疾病、癫痫、药物过敏、 白细胞减少史及严重的慢性衰退症状等。随机将60例入组患者分为研究组30例其中男18例女12例平均年龄± 平均病程±。对照组30例其中男17例女13例平均年龄± 平均病程±。两组一般资料经χ2检验均无统计学意义。
