5SiteInspectionWhenthePermitIssuerissatisfiedtheExcavationRequestandSupportingDocumentationisadequate,thePermitIssuermustconductasiteinspectiontoensurethattheagreedcontrolsintheSupportingDocumentationhavebeenimplementedonsite.
NOTE:ThePAPLRepresentativemustensurethattherequirementsofthisGuidanceNoteareappliedinrelationtoExcavationSafetyRules.

6IssuePermitOncetheSupportingDocumentationhasbeenreviewedandthesiteinspectioncompletedthePermitIssuermustsignandissuetheExcavationPermittoenableworktocommence.

7MonitoringSpotChecksmaybeconductedtoensuretheagreedconditionsoftheExcavationPermitarebeingimplemented.

8CompletionofWorkAftertheworkiscompletedthePermitApplicantisrequiredtonotifythePermitIssuerandthePAPLRepresentativethattheworksarecomplete.

1GeneralRequirementsAHRA–EXCAVATIONPermitisrequiredforallExcavationswhichmayimpactonPerthAirport'sinfrastructure,services,operationsorpersonnel.
The'HierarchyofControl'mustbeappliedwhenplanningand/orconsideringcontrolmeasures.
Itistheresponsibilityofthecontractortoensurethattheworksareadequatelysupervised/inspectedtoensuretheworksareperformedasdetailedintheExcavationrequestandasexpected.
NoMechanicalExcavationcanbeundertakenwithin1metreofanyPerthAirportServiceunlessanagreedsafesystemofworkisinplaceandagreedwiththePAPLRepresentative.
HREZExcavationsinthevicinityofHighVoltageServicesrequiretheservicestobeisolatedwherereasonablypracticable.
ItistheresponsibilityofthecontractortoliaisewithPerthAirportElectricalTeamtoarrangetheisolation.
ThePAPLRepresentativemustbekeptabreastofanycommunicationswiththeElectricalTeam.
WheretreerootsareencounteredtheabilityforthesetodisturbServicesmanymetresawayshouldbeconsideredandappropriateplanningundertakentoensurethatremovaldoesnotcausedamage.
Priortocommencementofexcavationsaffectingroadsortrafficmovement,adocumentedtrafficmanagementplanshouldbepreparedthatincludes,wherenecessary,trafficcontroller,barricadesandanyroadclosures.

Step2–ExcavationSiteIdentificationTheextentandlocationoftheExcavationistobeaccuratelymarkedonthesiteusingtheinformationprovidedontheExcavationLocationPlanrequiredforanExcavationPermit.
Step3–ServiceLocationandMarkingRequirementsEachServiceshownontheDBYDdataandadditionalServicesidentifiedbythePAPLDesignOfficeistobemarkedonthesiteusingasystemofidentificationthatcanbemaintainedforthedurationofthework.
Identificationmarkers,suchasPVCpipes,sprayorsurveypegs,mustbeconsistentlycolourcodedandmustincludeinformationreferencingservicetype,pipe/conduitconfigurationandknownservicedepth.
DetailsofthecolourcodingmustbeincludedaspartoftheExcavationRequest.
HREZServicesrequireanincreasednumberofmarkerstobeinstalledandgreatercontrolonmarkerintegrityandlocationaccuracy.
TheServiceLocationContractoristoverifythateachServiceshownontheDBYDdataandadditionalServicesidentifiedbythePAPLdesignofficehasbeenmarkedonthesiteandthatthemarkersforeachcanbeclearlydistinguishedfromoneanother.
ServiceCrossingExcavationWhereExcavationcrossesaServicethemarkersaretoextendsufficientlybeyondeachendoftheExcavationtoensurethatatleasttwomarkersremainoneachsideoftheExcavationsothatthealignmentacrosstheExcavationcanbere-establishedatanytimeduringExcavation.
ServiceParalleltoExcavationWhereServicesrunparalleltotheExcavation,markersshouldbeinstalledatregularintervalsalongtheentirelengthoftheExcavationandextendingatleasttwomarkersbeyondtheextentoftheExcavation.

Step5–MarkerCurrencyItistheresponsibilityofthecontractortoensurethattheactivitiesonsitedonotdisturbmarkers.
IfmarkersaredisturbedtheServiceLocationContractormustre-confirmtheaccuracyofmarkersrelevanttotheremainingExcavationworksbeforetheExcavationworksrecommencing.

4InformationonBuriedServicesOwned/ManagedbyInfrastructureCompaniesMuchoftheburiedservicesinfrastructureonthePerthAirportEstateisownedandmanagedby3rdpartyinfrastructurecompanies,including:FuelLines–AirBPGasMains–WestNetEnergy,AlintaGasandtheDampiertoBunburyNaturalGasPipelineAuthorityTelecommunications-TelstraPlanningforExcavationinthevicinityoftheseservicesshouldincludeengagementwiththeseorganisations(duringtheplanningphase),includingtheidentificationofcontrolsoracontingencyplaninthecaseofHREZ.

3ServiceProtectionOfficerForeveryExcavationactivityonthePerthAirportestateapersonwithinthecompanyresponsiblefortheExcavationistobeidentifiedastheServiceProtectionOfficer(SPO).
TheSPOisthepersonthatPAPLholdsabsoluteresponsibilityforensuringthatnoServicesaredamagedduringtheExcavation.
TheSPOisspecificallyresponsible:ForensuringthatthecommitmentsmadeinandtheconditionsattachedtotheExcavationPermitfortheExcavation,arefollowedtotheletter;Forassessing,inatimelymanner,whentheidentifiedmitigationmeasurescannotbestrictlyfollowed;ForassessinginatimelymannerwhentheidentifiedmitigationmeasureswillnotachievethegoalofpreventingdamagetoServices;ForensuringthesafetyofpersonnelinvolvedorthosenearanExcavation;Forcausingtheworkstobehalteduntilthenecessaryre-assessmentofriskandsafetycanbeproperlyundertaken;FormanaginganychangesmadetotheExcavationPermitandtohavetheExcavationPermitre-approvedbythePermitIssuer.
TheSPOisresponsibleforensuringthecontinualprotectionofnewandexistingServicesduringactivitiesfollowingtheExcavationincludinginstallationofitemsinanyExcavation,thesurveyingofinstalledandexposedServicesandtheoperationofbackfilling.

4ControlofHighVoltageServicesWhenExcavationistobeundertakeninthevicinityofHighVoltageServicesthathavenotbeenisolated,aPAPLElectricalRepresentativenominatedbyPAPL'sAirportElectricalEngineermustalwaysbepresentduringtheExcavation.

5ExcavationSafetyWhereexcavationsare1.
5metresdeeporgreaterthecontractormustensure,thattheworkareaissecuredfromunauthorisedaccessbyprovidingsuitablesolidbarriersandsuitablesignage.
Theriskofinjurytoanypersonarisingfromthecollapseoftheexcavationsmustbeminimisedbyensuringthatallsidesoftheexcavationareadequatelysupportedbydoingoneormoreofthefollowing:oshoringbyshieldingorothercomparablemeans,obenching,obattering.
Nounauthorisedpersonshallbepermittedtoenteranexcavationwhileexcavationequipmentisoperating.
Toenablesafeaccessandegresstoandfromexcavations,ladders,rampsorothersafemeansofentryorexitshouldbeprovided.
Safeandsuitablemeansofmovementmustbeallowedforpersonsinandaroundandexcavatedarea.
Noitemofplantorexcavatedmaterialshouldbeplaced/storedneartheexcavatedarea.
Considerationshouldbegiventotheneedforanareatobeestablishedforstockpilingexcavatedmaterial.
Ifitisnotreasonablypracticabletoestablishastockpilearea,excavatedmaterialshouldbeplacedoutsidea45-degreeslopelinepassingthroughthebottomoftheexcavationandinnocasecloserthan1000mmfromtheedgeoftheexcavationExcavationsshouldbeinspecteddailybyacompetentpersonbeforethestartofworks.

6EnvironmentalandHeritageClearanceThecontractormustensurethatHeritageMonitorsareengagedduringExcavationworksinareasofremnantvegetationorareasformallyregisteredbytheStateasheritagesites.
Shouldarchaeologicalmaterialbedisturbedduringexcavations,thecontractormustceaseallExcavationworksandcontactthePAPLRepresentativewhowillnotifyPerthAirport'sApprovalsTeam.
AClearing/PruningPermitmustbesoughtforanyremovalofvegetation,whethernativeorlandscapetrees/shrubsindevelopedareas.
ACleanFillFormmustbeapprovedbytheEnvironmentTeambeforeimportingoffillormovingsurplusspoilwithintheestate.
ADewateringPlanmustbeapprovedbytheEnvironmentTeambeforeanyExcavationworksrequiringdewatering.
