1General:TheCityofMesarequiresthatallconstructionmaterialstobesupplied,constructedorinstalledin,onoracrossanyCityofMesaeasement,right-of-wayorCity-ownedpropertybesubjecttoinspection,qualitycontrol(QC)&qualityassurance(QA)testing,andapprovalorrejectionbytheCity.
Thisrequirementalsoappliestoanymaterialsorinfrastructurethat,oncein-place,willbeowned,operatedormaintainedbytheCityofMesa,regardlessofwheretheyareinstalled.
AnymaterialrejectedbytheCityofMesashallberemovedimmediatelyandreplacedinanacceptablemannertotheCityatnoadditionalcosttotheCity.
"QualityControl(QC)Testing"istestingperformedtoassurethematerialsinstalledcomplywiththerequirementssetforthinCitystandardsandtheContractDocuments.
"QualityAssurance(QA)Testing"istestingperformedtoverifytheaccuracyandapplicabilityoftheQCtestingresultsandtoascertainthatthematerialsinstalledmeetthespecifiedlevelsofqualityinaccordancewithCitystandardsandtheContractDocuments.
ForCityofMesaprojects(wheretheCityofMesaistheContractingAgency),theGeneralContractoremployedbytheCityshallberesponsibleforperformingtheQCtestingaspartoftheContractWork.
ForprojectswheretheCityofMesaisnottheContractingAgency("non-Cityprojects"),thepersonorentityholdingtheCityofMesapermitforthework("Permittee")isresponsibleforcompletingtherequirementssetforthherein,asaconditionofthepermit.
For"non-Cityprojects",theverbiageofthissectionpresumesthatthePermitteeeitheristheGeneralContractorperformingthepermitworkorthatthePermitteewillcontractuallypasstheQCtestingresponsibilityontothePermittee'sGeneralContractor.
Hence,theword"Contractor"isusedthroughoutthissectionwhenspeakingoftheentityresponsibleforQCtestingrequirements.
Itshallbeunderstood,however,thatfor"non-Cityprojects"theseresponsibilitiesareultimatelyincumbentonthepermitholder.

TheCitymayalsoelecttoperformindependentQAtestingforanyprojectorpermit.
TheCitywilltypicallyperformQAtestingatafrequencyofapproximatelyoneQAsampleforeveryfourQCsamples.
However,theCitymaydeviatefromthisfrequency(eithertoincreaseordecreaseit)attheCity'sowndiscretion.
TheContractor(whethertheCity'sContractororthePermittee'sContractor)shallprovideallsupportnecessarytoperformQCandQAtestingandsampling(i.

shoringfortesting,trench,backfill,backhoes,motorgraders,loaders,etc.

2QualityControlTestingProgramRequirements:TheContractorshallestablish,provide,andmaintainaneffectiveQualityControlTestingProgram(QCTP).
TheContractorshalldevelophisownprogramorprocuretheservicesofaconsultant.
Ineithercase,thepartyperformingthetestsshallbecurrentlycertifiedbytheNationalBureauofStandardsintheNationalVoluntaryLaboratoryAccreditationProgram(NVLAP)forconstructionservicesortheAASHTOAccreditationPlan(AAP)forSoils,AsphaltandConcrete.
AlltestingshallbeunderthedirectionofaProfessionalEngineerregisteredintheStateofArizona,knowledgeableinMaterialsTesting.
AlltestreportsandformsshallbestampedbytheEngineer.
Allpersonnelperformingmaterialssampling,collectionand/ortestingshallbeproficientintheirassigneddutiesandpossesscertification(s)commensuratewiththeirpositionandresponsibilities.
Theminimumcertification(s)foreachtechnicianshallbeNICETLevelII,ArizonaTechnicalTestingInstitute,AmericanConcreteInstitute,orothernationallyrecognizedprogramapplicabletotheprojectandapprovedbytheCityofMesa.
AllpersonnelperformingfieldtestingorsamplingshallcarrycopiesoftheircertificationswiththeminthefieldandshallproducethemuponrequestfromtheCityInspector.
FailuretoproduceacceptabledocumentationofpropercertificationbyeitherfieldpersonnelorthelaboratorymaybegroundsfortheCitytorejecttestingorsamplingdonebythatentity.
Priortothestartofanyconstruction,theContractorshallgivetheCityInspectorascheduleoftheproposedtestingandthenameofthelaboratorythatwillperformthework,alongwithevidenceoftherequiredcertificationsrequiredherein.
Iftheproposedschedule,laboratoryorpersonnelaredeficient,theCitywillnotifytheContractorandworkmaynotproceeduntilthedeficiencyiscorrected.
Atanypointduringtheproject,iftheCitydeterminesthattheQCactivitiesdonotcomplywiththerequirementssetforthherein,theCitymay:(1)OrdertheContractortoreplaceineffectiveorunqualifiedqualitycontrolpersonnel.
(2)OrdertheContractortostopoperationsuntilappropriatecorrectiveactionistaken.

Uponreceiptofthesedocuments,theInspectorwillobtainthenecessaryapprovalsandforwardtheApplicationtotheappropriateCitystaffforpaymentprocessing.
Theprogresspaymentwillbeprocessedforpaymentwithinfourteen(14)days(exceptfinalpayments)aftertheApplicationforPaymenthasbeencertifiedandapprovedbytheEngineerinaccordancewithA.

Portablevibratorsor"stingers"shallonlybeusedtosupplementinternalvibratorsonthemachineandnotasasolesourcetoconsolidateanddistributetheconcretemix.

TheEngineershallrequireremovalandreplacementofthosesectionsofAugust2017Page19of24pipelinewhichtheEngineerjudgestobenon-repairableorwhichisnotwithinrequireddimensionaltolerancesincludingalignmentandgrade.
Whenconcreteplacementisdonebyamethodrequiringtheuseofmetalinnerforms,theContractorshallschedulehisworkforce,byextended,staggeredormultipleshifts,asrequired,toprovideforremovalofformswithin4to6hoursofplacementofconcreteandstartofrepairing,patchingandfinishingofpipelinetoconformwithspecificationrequirements.
Whenconcreteplacementisdonebymethodsusingpneumaticallyinflatedinnerliner,theContractorshallschedulehisworkforce,byextended,staggeredormultipleshifts,asrequired,toprovideforremovalofthepneumaticinnerlinerwithin12hoursofplacementofconcreteandstartofrepairing,patchingandfinishingofpipelinetoconformwithspecificationrequirements.
Allrockpockets,non-longitudinalcracksorindentationsshallbecleanedout,moistenedandfilledwith1:2cementgroutorapprovedepoxymaterial.
Exceptwhere,intheopinionoftheEngineer,thewidthand/orlengthofthecrackmayindicateastructuraldeficiency,repairsshallbemadeasrequiredforlongitudinalcracks.
AtthediscretionoftheEngineer,longitudinalcracksexceeding0.

Alongitudinalcrackshallbedefinedasonewhichhasthegeneraldirectionofa30degreeangleorlesswiththealignmentofthepipe.
Irrespectiveofconcreteplacementmethod,allrepairs,patchesandfinishingshallbecompletedwithin24hoursofconcreteplacement.
TheContractor,priortostartofconcreteplacementonprojectshallsubmitawrittenscheduleofhisproposedworkactivitiesandworktimeschedulesfortheEngineer'sreviewandapproval.
