COACHING GROUP AGREEMENT

This coaching agreement between (the coach) Betsey Wi l l iams,M.S. and (theclient) beginson forsix(6) group coachingsessions. Furthersessions may be agreed upon after this initial contract.

Coaching is a sequential processconsisting of clarifying your intentions, designinggoals and taking action within a safe, supportive relationship. Coaching is notpsychotherapy, norshould it be used as a substituteforcounsel ing, legal, financial,or medical services.

Confidential ity:

Al l information disclosed in group sessions is confidential and may not be revealed toanyone without written permission. Thecl ient understands and gives permission forthecoach toconsult with a mentorcoach at thecoach’s discretion.

Sessions:

Group sessions wi l l be 120 minutes each with breaks as needed.

Contact between Sessions:

There wi l l be no charge forone phone cal l between sessions under 10 minutesand/orone e-mai l . If more time is needed, this may be scheduled by mutualagreement and a pro-rated fee wi l l be charged.

Fee and Payment for Services:

The total fee for six group sessions is $300.00 plus a $10 materials fee. The $50.00deposit wi l l be credited toward the total amount. The fee may be paid monthly orper session. If you cannot attend a session,you are sti l l responsible for the fee. Ifyou wish to receive CEU’s, the processing fee is $10.
